New Young Carers Referral Form Launches

9th March 2026

Carers Support has launched a new Young Carers referral form, designed to make referrals clearer, simpler and more effective.

This updated form will help referrers and our team gain a clearer understanding of a young carer’s situation and identify what support they may need. The form is straightforward and similar in style to those used for Early Help, helping professionals feel confident and familiar when completing it.

The form is available as an online form on this website, making it easy to access and submit.

Supporting completion

For ease of completion, there is also a downloadable set of questions. This can be used when talking through the referral with a family and/or young carer, allowing you to gather all the necessary information before submitting the form online.

Important update

Please note: from this point onward, we will no longer accept previous versions of the Young Carers referral form.

What professionals are saying

“The form is really useful to gather the necessary information to ensure a comprehensive understanding of a young person and their family’s circumstances, so that the best support can be put in place to meet their individual needs. It has the potential to open up some really insightful conversations with families, which are helpful for schools and empowering for young carers themselves.”
— Stacey Gwynn, Young Carers Lead (Post-16), Cabot Learning Federation

“The updated version of this form enables you to gather and record all of the information needed to make a successful referral for a young carer in straightforward and simple steps.”
— Jason, DDSL and Young Carers Lead, Winterbourne Academy
